I understand that the permanent jewelry experience involves microwelding and that protective equipment must be worn during this time.

I understand that permanent jewelry can easily be removed.

I understand that all White Sands Co. permanent jewelry is final sale and once any chain is cut, no return policy is applicable to any product purchased or service rendered.

I understand that this jewelry can break and/or stretch if pulled or caught and that White Sands Co. is not responsible.

I certify that I have adequate insurance to cover any injury or damage arising out of the products purchased or services rendered, and I agree to bear the costs of such injury or damage not otherwise covered by my insurance. I further certify that I assume the risk of any medical or physical condition I may have, whether known or unknown.

I understand that the use of my photos may be used for marketing purposes and may appear online.

I certify that I am at least eighteen (18) years old and am fully competent to give my consent or I have the consent of my parent/guardian if I am a minor.
